Model Release Form

I do hereby give Peaceful Valley Farm Supply, Inc., and parties designated by Peaceful Valley Farm Supply, Inc., including but not limited to clients, licensees, purchasers, agencies, and periodicals, the irrevocable right to use the model’s name, image, likeness, voice, video recordings, and photographic portraits in which the model may be included, in whole or in part, or composite or distorted in character or form, without restriction as to changes or alterations, in conjunction with the model’s own or a fictitious name, or reproductions of such photographs in color or otherwise, made through any medium at Peaceful Valley Farm Supply, Inc. or elsewhere, and in any and all media now or hereafter known, including the internet, for art, advertising, trade, or any other purpose whatsoever. I also consent to the use of any published matter in conjunction with such photographs. I specifically consent to the digital compositing or distortion of the portraits or pictures, including without restriction any changes or alterations as to color, size, shape, perspective, context, foreground or background. I as the model, or parent/legal guardian of the model, have read this release and fully understand its contents.

I hereby waive any right that I may have to inspect and approve the finished product or copy that may be used in connection with an image or video that was taken of the model or the use to which it may be applied.

I further irrevocably release Peaceful Valley Farm Supply, Inc., or others for whom he/she/they is/are acting, from any claims for remuneration associated with any form of damage, foreseen or unforeseen, associated with the commercial or artistic use of these images unless it can be proven that said reproduction was maliciously caused, produced, and published for the sole purpose of subjecting me to conspicuous ridicule, scandal, reproach, scorn, and indignity.
